NOTICE OF PROCEEDINGS FOR TERMINATION OF PARENTAL RIGHTS, APPLICATION FOR ORDER DETERMINING CHILD ELIGIBLE FOR ADOPTION WITHOUT CONSENT OF NATURAL FATHER AND APPLICATION TO FINALLY ADOPT MINOR CHILD

NOTICE IS HEREBY GIVEN that proceedings for termination of your parental rights of the Minor Child Z.W.D., and for order determining said child eligible for adoption without the your consent, has been filed in the District Court, Rogers County, Oklahoma, alleging that Minor Child Z.W.D., born on 15 March, 1999 in Tulsa, Oklahoma was born out of wedlock and requesting that the parental rights of the you, the alleged natural father be terminated due to your failure to maintain a positive and substantial relationship and/or provide support for the minor child for at least 12 of the 14 months immediately preceding the commencement of this action and that an order be entered determining said child eligible for adoption without consent of the natural father, all as more fully stated in the Petition and Application For Adoption on file in the Office of the District Court Clerk, Rogers County, Oklahoma, by Petitioner JEREMY MICHAEL PAINTER.
NOTICE IS FURTHER GIVEN that the hearing on the issue of whether the requested adoption can be granted without the consent of ANTHONY JASON HAWKINS will be heard before the Honorable J. Dwayne Steidley at the Rogers County Courthouse, Claremore, Oklahoma on the 20 day of June, 2006 at 10:00 a.m.
NOTICE IS FURTHER GIVEN that, if after the above stated hearing is conducted, the Court finds the consent of said alleged natural father is not necessary, there will be a hearing before the Honorable J. Dwayne Steidley at the Rogers County Courthouse, Claremore, Oklahoma on the 20 day of June, 2006 at 10:30 a.m. to determine the final merits of Mr. Jeremy Michael Painter’s Petition For Adoption, which will include the issue of whether it is in the best interest of the minor child to be adoption by said Petitioner.
NOTICE IS FURTHER GIVEN that at the above hearings, you are entitled to the opportunity to be heard, including the right to object to said proposed termination of your parental rights, the proposed order determining said child eligible for adoption without our consent and the proposed final adoption of the above-named child. You are further granted the right to legal counsel and, at the hearing, to offer evidence in support of your objection(s), if any, all as provided by law.
NOTICE IS FURTHER GIVEN that failure by you to appear at said hearings shall constitute a denial of your interest in the child which denial may result, without further notice of the proceeding or any subsequent proceeding, in the termination of our parental rights and in the transfer of said legal parental rights and the child’s care and custody to the proposed adoptive parent.
